Checks should be made payable to CAEA and may only be submitted with this form via physical mail.  When emailing the form, you must pay by credit card.  All international payments must be paid by credit card only.


Please indicate ALL events you plan to attend or for which you plan to purchase tickets. No ticket will be issued for any unmarked event. NOTE: Payment made with checks must be postmarked by October 27, 2017, in order to be processed at the Early Bird rate. Credit card payments must be received by October 27, 2017, in order to be processed at the Early Bird rate. Additionally, all mail-in, emailed, and online registrations must be received no later than November 1; after that date, registration will only be available at the conference.

Free master class drawing: Conference registration paid in full by September 15 qualifies for a free master class drawing.

Cancellation Policy: If you cancel on or before October 22, 2017, you be charged a $50 cancellation fee.  After October 22, 2017, no refunds will be issued unless an event is cancelled.
